    Please print this document, fill it out and send with other application material.

    Both Masters and Ph.D. students may apply for support. Please take a moment to look through our Graduate Teaching Assistantship (GTA) Information at http://www.math.ksu.edu/main/graduate/assistantship.htm

  2. Statement of Academic Objectives.

  3. Three letters of recommendation. There is no required form for these letters.

  4. Two original sets of all transcripts [certified list of coursework] from all universities and colleges attended. If not a U.S. university, be sure these include your name, name of school attended, title of courses, dates (semester/year) taken, marks received and an official seal and/or signature of authorized faculty from that school.

  6. The application may be filled out online and submitted electronically, but the signature page must be printed, signed and mailed in.
  7. An Application fee of $30 is required by the Graduate School. Please make payment out to: `KSU Graduate School'.To pay the application fee electronically, you must fill out your application on-line. If you want to be advised by the Department prior to the payment of this fee, please submit items 1 through 5 above and we will be happy to do an informal review.

    The GRE is not required but you may provide your score if you wish.
    Kansas State University ETS reporting code is: 6334.Mathematics code for GRE is 0703.

  • Deadlines: Applications may be submitted at any time but the following guidelines if you wish to be considered for support are as follows:

    1. February 1st for the following fall semester
    2. September 1st for the following spring semester
